La protéine PCSK9 régule l’activité des LDL-récepteurs.
Les mutations gain de fonction de PCSK9 sont une des causes génétiques d’hypercholestérolémie familiale. À l’inverse les mutations perte de fonction sont associées à un taux abaissé de LDL-cholestérol et à une protection coronarienne.
Les anticorps monoclonaux inhibiteurs de PCSK9 sont très efficaces pour abaisser les taux de LDL-cholestérol, avec un excellent profil de tolérance et de sécurité.
Deux anticorps monoclonaux humains, alirocumab et évolocumab, ont démontré, en addition à un traitement par statine, un bénéfice cardiovasculaire chez des patients en prévention secondaire.
Un récent consensus européen a précisé les patients candidats à une inhibition de PCSK9, à savoir les patients en prévention secondaire et les patients avec hypercholestérolémie familiale en prévention primaire qui conservent un taux de LDL-cholestérol franchement augmenté malgré un traitement par statine à dose maximale tolérée avec ou sans addition d’ézétimibe.

PCSK9 protein is a key regulator of LDL receptor activity.
Gain-of-function mutations in PCSK9 are one of the genetic causes of familial hypercholesterolemia. Conversely, loss-of-function mutations are associated with lower levels of LDL cholesterol and reduced coronary heart disease.
Monoclonal antibodies targeting PCSK9 are highly efficacious in lowering LDL-C levels, with a good tolerability and safety profile.
Two PCSK9 inhibitors, alirocumab and evolocumab, have demonstrated a cardiovascular benefit in addition to statin therapy in patients with established cardiovascular disease.
A recent European consensus has defined the candidates for PCSK9 inhibitors, e.g., patients with established cardiovascular disease and patients with familial hypercholesterolemia in primary prevention, with substantially elevated LDL-C levels despite maximally tolerated statin with or without ezetimibe therapy.
